UKGTN Diagnostic Members - Gene Dossier or additional provider form

Before completing a Gene Dossier please check whether the test you would like to provide is on the NHS Directory of Molecular Genetic Testing.

• If the test is not on the NHS Directory of Molecular Genetic Testing then please complete a full Gene Dossier.
• If the test is already on the NHS Directory of Molecular Genetic Testing please complete an Additional Provider Form. Please note that the UKGTN may request further details or completion of a full gene dossier if it is felt that it is required. We would request that laboratories collaborate with existing providers to develop consensus testing criteria.
• To change the service level of a test the providing laboratory should contact the UKGTN project team in writing with the proposed change.

Before completing a Technical Service Provider form please check whether the test you would like to provide is on the NHS Directory of Molecular Genetic Testing.

• If the test is not on the NHS Directory of Molecular Genetic Testing then please contact the UKGTN project team for information on how to proceed with the application.
• If a test is already on the NHS Directory of Molecular Genetic Testing please complete the Technical Service Provider form.
• To change the service level of a test the providing laboratory should contact UKGTN project team in writing with the proposed change.
